SMMS CRUSADER REGISTRATION DAYS                                           


Parent Packet

  • All Registration Packet forms will be available online at beginning on July 1st. Click on “2019 Registration Packet” and print all necessary forms.Fill them out before your registration date and bring them in during your assigned day and time.
  • Registration packets will also be available for pick up in our office between August 2nd and August 6th

 from 8:00am to 2:00pm.


Registration Dates and Schedule:

  • 6th grade students – Wednesday, August 7th
  • 7th grade students – Thursday, August 8th
  • 8th grade students – Friday, August 9th


Last Name, A – L – 8:00am to 10:30am

Last Name, M – Z – 12:00pm to 2:30pm

*Parents with more than one student in different grades can attend on Friday, August 9th to register them all at once.

Registration Day Agenda:

  1. TDAP Verification-7th graders are required to be immunized with a pertussis (whooping cough) vaccine called TDAP. Your child will not be allowed to start school without it.
  2. Residence Verification-The residency of the parent/guardian/caregiver and the student must be verified by one of the following current (within 60 days) items from each category. Must include residency verification form enclosed in the packet.
    • Category One:
      • Current Property Tax statement or payment receipts, mortgage statement or payment receipts.
      • Current Rental Property contract, lease, or payment receipts that have your name, date and must be dated within the last two months (60 days) of your verification date.
      • Escrow papers for a new home
    • Category Two:
      • Current Utility Service contract, statement or payment receipts (water, electric, trash, cable). _Phone/Mobile Phone contracts are NOT accepted.
      • Current pay stubs with address
      • Voter registration
      • Current correspondence from a government agency
      • If you reside with another family, a notarized affidavit of residence must accompany enrollment.

  4. Purchases: P.E. Clothes and Yearbook
  • Registration will be the time to make your student purchases. Yearbooks are $40.00 during registration and $45 beginning September 9th, and P.E. uniforms are $15.00.Payment can be made by cash or check only. Exact change is appreciated and no bills over $20.00 will be accepted. You can also purchase these items online at with your credit card. Log on to “Web Store Access”.
